Payment
Payment is expected at time of
service.
We accept cash, check, debit, MasterCard, Visa, Discover and American Express.
Insurance
We file insurance claims on your behalf – usually before you leave the office – and will make inquiries as needed, promptly and efficiently.
Most patients receive a check from their insurance company in one to four weeks.
If you do not, please contact us.
Discounts
We offer a 5 % courtesy discount on amounts exceeding $500, if paid in full by check or cash on the initial day of service or treatment.
Credit

Cancellations
Should you need to cancel an appointment, please provide 24 hours’ notice. The second appointment broken without proper notice will result in a charge of
$50.
